The practice welcomes new patients who live in our catchment area. When registering, patients are asked to complete a questionnaire and to bring in proof of address i.e. a recent (within 3 months) utility bill or bank statement. We prefer that all the other members of your family join the practice at the time of the initial registration. We believe that good primary medical care depends on the doctors knowing the whole family and its problems. We expect all cervical smear tests and immunisations to be undertaken at our surgery. Please allow at least 2 working days for your registration to be processed with NHS England, before you can book an appointment access our services.
Below are two forms required to register and the registration checklist. Please return the completed forms to the surgery with the required information.
